Lower eyelid lift in Zurich
The lower eyelid region is anatomically demanding. What is perceived as an eye bag almost never arises from a single cause — and here, more than elsewhere, planning determines the result.
At NP Aesthetics, planning is carried out by Dr. med. Michael W. Payne. Consultations and surgery currently take place at Klinik Selnau.
What causes dark circles and shadows under the eyes
A lower eyelid lift that only removes fat therefore falls short. What matters is which of these components create the visible findings.
Excess skin
Fine creasing and loose skin, influenced by elasticity and sun damage.
Fat prolapse
The fat compartments behind the orbital septum bulge forward and cast a shadow.
Tear trough
The hollow at the transition to the midface arises from tissue anchoring to the bone and from volume loss beneath it.
Lid tension
Reduced tension of the lower lid alters the lid position and increases the risk of malposition after surgery.
Descent of the midface
When the midface moves downwards, the lid region appears longer — even without relevant fat prolapse.
Surgical approaches
Transconjunctival approach
Access via the inner surface of the lid, with no external scar. Suitable when fat prolapse is the main concern and skin and lid tension are good.
External approach
An incision just below the lash line. Additionally allows treatment of excess skin and work on deeper layers.
Fat preservation and repositioning
Instead of simply removing fat, it can be moved into the tear trough to smooth the transition to the midface. Whether this is useful depends on the findings.
Canthal support
Where lid tension is reduced, the outer corner of the lid is tightened or stabilised to prevent lid malposition.
Relationship to the midface
If the midface is substantially involved, a lift or volume addition may be needed in addition; the lid operation alone will not then achieve the goal.
Consultation and examination
Skin quality, fat distribution, lid tension using simple clinical tests, the position of the outer corner of the lid, the midface and previous ophthalmological findings — particularly dry eyes — are assessed. Standardised photographic documentation is part of this.
Anaesthesia and surgery
Depending on the extent, the procedure is performed under local anaesthesia with sedation or under general anaesthesia. The duration depends on the approach and any combinations, for example with an upper eyelid lift.
Recovery
Bruising and swelling are marked in the first few days and subside over one to three weeks. Cooling, an elevated head position and avoiding bending and straining support recovery.
Office work is usually possible after about a week, and patients are often socially presentable after two to three weeks. Contact lenses, make-up and sport only after medical clearance. The final result becomes apparent after several months.
Risks and limitations
Lid malposition
Sagging or outward turning of the lower lid is the most relevant complication. As a precaution, lid tension is tested and stabilising measures are planned where necessary.
Dry eyes
A temporary feeling of dryness is common; with pre-existing conditions it can last longer.
Asymmetry
Differences between the sides in swelling, contour or lid position are possible and cannot be entirely avoided.
Further risks
Postoperative bleeding, infection, wound healing and scar problems, over- or under-correction, remaining shadows. Very rare serious complications are discussed during the informed consent consultation.
Revision
Corrections are possible after healing is complete; the timing is determined individually.
Costs and personal consultation
The medical consultation for surgical procedures costs CHF 250. Following the examination, you will receive an individual cost breakdown for the recommended procedure. This should clearly state which services are included.
Questions worth asking before surgery
Are eye bags simply excess fat?
Only partly. Bulging fat is one component; skin condition, lid tension, the tear trough and a descended midface also play a role. If only fat is removed, the region can look hollow and the shadow may remain.
How long does recovery take?
Bruising and swelling are usually more pronounced on the lower lid than on the upper lid. Many patients allow two to three weeks until they are socially presentable; subtle residual swelling can persist longer.
Will there be a visible scar?
The transconjunctival approach leaves no external scar. With the external approach, the scar lies just below the lash line and is usually inconspicuous once the redness has subsided.
What is a canthopexy and when is it needed?
It tightens or stabilises the outer corner of the lid. It is considered when the lower lid has reduced tension, to prevent lid malposition after surgery.

A good decision begins with a conversation.
The initial consultation is about clarification, not selling. It does not necessarily end with a surgery date — often it brings the realisation that a different approach, or no procedure at all, is the right choice.
