Lower eyelid lift or fat transfer: Distinguish between bulging and lack of volume
Published
Lower eyelid surgery can treat excess skin or bulging fatty tissue if the findings are suitable. A fat transfer supplements the body's own fat in selected areas. A depression under the eye requires different planning than a bulge; both can exist at the same time.
“Dark circles” is a description, not a diagnosis. Shadow, pigmentation, skin quality and contour can influence the impression. No single treatment automatically solves all of these questions.

What actually bothers you?
You may see a small bulge under the eye. Maybe the transition to the cheek looks sunken. Or you want a change in fine skin wrinkles. During the consultation, be as specific as possible about which area you mean.
The timing is also important: Is the impression permanent or does it fluctuate? Are your eyelids swollen in the morning? Are there any problems with your eyes? Such information helps to distinguish aesthetic desires from questions that require other medical clarification.
What lower eyelid surgery can change
Depending on the findings, skin and fatty tissue can be treated. This involves checking whether fat should be reduced or relocated within the region. Lower eyelid surgery does not automatically mean removing as much tissue as possible.
The stability of the lid and secure contact with the eye are essential. Previous operations, dry eyes and existing eyelid misalignments influence the planning. Further ophthalmological evaluation may be required before an aesthetic recommendation is reasonable.
What fat transfer can do
During an autologous fat transfer, fat is removed from a suitable part of the body and prepared for treatment. It can be an option for selected volume deficits. The amount that remains in the long term is not entirely predictable; further treatment steps may be necessary.
Your own fat does not remove excess skin and does not automatically correct an unstable eyelid margin. Ask which hollow it is intended to fill and what limits there are. The extraction site and possible complaints are also part of the explanation.
What role do other causes play?
Not every dark impression under the eyes is caused by a lack of volume. If pigmentation or other skin changes are of concern, a different professional assessment may be required. Surgery or filling should not be chosen simply because the collective term “dark circles” is used.
Put your most important goals in order. In this way, the consultation can explain which change could be addressed first and whether some residual change is to be expected. Phased planning can make more sense than several measures at the same time.
Risks and limits
With lower eyelid surgery, the possible symptoms include bleeding, infection, dryness, changes in the position of the eyelids and difficulty closing the eyelids. Rare, serious complications, including visual impairment, should be included in personal risk discussion.
Autologous fat can cause bumps, asymmetry, lumps or fat necrosis. Even injections on the face are not free from rare, serious vascular complications. “Body-owned” therefore does not mean “risk-free”. Suitability and concrete implementation are crucial.
What you can collect before the appointment
Note previous treatments in the eye area, fillers used and surgeries. Mention eye discomfort and eye drops. A photo can show the progression; However, information about the time and symptoms are just as important.
Ask about everyday recovery: Visible swelling, contact lenses, screen work and planned travel should be discussed for your specific procedure. A blanket release from a general article would not be sufficient.
Frequently asked questions
Are fat transfer and fat repositioning the same thing?
No. A transfer introduces fat from another site; repositioning uses fat already present within the surgical area.
Can your own fat get rid of all dark circles under the eyes?
No. The impression can have several causes that require different approaches.
Is a combination possible?
For selected findings, yes. Each step should have a clearly explained task.
