Facelift or filler: differentiate volume and tissue position
Published
If indicated, fillers can add volume and change selected contours. A facelift treats descended tissue and excess skin. Both procedures have different tasks and cannot be used interchangeably. Which option is right depends on your face and the desired result.
A wrinkle alone does not say which treatment is appropriate. It is also crucial whether volume is missing, tissue has sunk or several changes are occurring together.

What change concerns you?
Maybe the cheek looks less full. You may be bothered by the contours along the lower jaw. Or you want a certain wrinkle to be less pronounced. First formulate your wish without the treatment name.
It is helpful to ask: Do I want to have something filled or change the position of the tissue? You don't have to diagnose this yourself. However, the distinction gives the conversation a clear direction and prevents discussion only about injection volumes or technique names.
What fillers can do
Fillers are used for selected findings to treat missing volume or contours. Effect, durability and possible subsequent treatments depend on the material and the region. Not every filler is the same, and not every set of findings is suitable.
If there is significant excess skin or descended structures, additional volume does not necessarily achieve the desired goal. Therefore, ask what exactly needs to be refilled and why. Treatment through small entry points is not automatically a treatment without relevant risks.
What a facelift treats
A facelift can change the position of descended facial structures and excess skin. The scope is adjusted to the findings. Depending on the situation, the neck region is part of the planning; the eye region may require other measures.
A facelift does not solve every problem of the skin's surface and does not automatically replace missing volume. Even surgery does not stop the ageing process. The consultation should make it clear which areas can be improved and which changes remain separate.
Can both procedures be combined?
For selected findings, a combination may make sense. Then the individual steps and their sequence should be explained in a comprehensible manner. It may also be appropriate to treat one part first and wait for the result.
Inform the team about previous filler treatments: material, region and date, if known. Previous operations are also important. Bring existing documentation with you rather than trying to remember every detail.
Risks differ
Fillers can cause swelling, bruising, infection, lumps or other unwanted reactions. Accidental vascular injection can rarely cause serious damage, including vision loss or stroke. Therefore, suitability, experience and an appropriate medical approach are essential.
A facelift can cause postoperative bleeding, wound-healing problems, scars, altered sensation and nerve injury, among other complications. The individual risk discussion covers their frequency, significance and your personal risk factors. Differences in recovery time are also part of the decision.
What is often forgotten when comparing
Don't just compare the effort on the day of treatment. Ask about aftercare, possible repeat treatments, and how to deal with an adverse result. Costs should be explained as part of a concrete plan, not just as a blanket “injection vs. surgery” comparison.
Your willingness to change is equally important. A finding can allow for several justifiable paths. You can decide for a smaller change, to wait or not to undergo any treatment.
Questions for your appointment
- Which of my wishes concern volume and which tissue position?
- What would additional volume not improve in my case?
- What would be the scope of an operation and why?
- What follow-up treatments might be necessary?
- What risks differ between the specific options?
Frequently asked questions
Are fillers always the better first stage?
No. The first recommendation is based on the findings and your goals, not on a fixed order.
Can a facelift remove all wrinkles?
No. Not every wrinkle and every skin change is a facelift task.
Is a result possible without treatment?
You can decide against treatment at any time. A consultation does not obligate you to undergo an intervention.
